BP Logix - Where is my form in the workflow?

A step-by-step guide to find where your BP Logix form is in the process, or who is currently assigned your form.

Where is my form in the workflow? 

Log in to BP Logix at For questions logging in, see KB114299.

Once logged in, your home dashboard will show any forms you have available to submit, any open/active tasks you may have, as well as previously submitted forms. If you want to know where a previously submitted form is in the process, click My Submitted Forms at the top of the screen. 


On this page, you will see all submitted forms, both active and completed, the date submitted, and the current workflow status. You may also filter by the date the form instance was created on this page.  


Find the form you are looking for the status of and select the form name. 


This will pop up a new window and open the form again.  

Please note: If new window does not open, ensure that you have pop-up blockers turned off. 

Scroll to the bottom of the form and you will see the routing slip. The routing slips shows who the form is actively assigned to complete. It will show the date and time that the form task was assigned to this user. 


Although you cannot edit completed forms, you will still be able to see the routing slips on completed forms. Again, select the form name. 


Scroll to the bottom of the form to see the completed routing slip.
