Business Services Form - SFO Electronic Deposit Agreement

The SFO Electronic Deposit Agreement Form is completed to request the electronic deposit of SFO revenues to Business Services, via Venmo, for student faculty organization revenues. The policy related to this form can be found on the Business Services forms webpage as SFO Electronic Deposit Policy. It is intended that this form be completed by the president of the submitting SFO.

Instructions for Completing the Form via DocuSign:

Upon opening the above link to the SFO Electronic Deposit Agreement Form, the president will be prompted to enter their name and email address within the fields provided for the SFO President role. Once the role fields are populated, select Begin Signing and you will be directed to the form itself. After accessing the form and populating the provided fields, select Finish to submit the form to Business Services.
