How to create a new EAF

Navigate to the BPLogix Homepage https://uwlax.bplogix.net/workspace.aspx

Select “Employee Action Form (EAF)”

Select “Type of Action” and “Action Reason”

Enter all required fields (fields marked with a red line)

Click “Submit”

The submitter will receive an email (example below) from BPLogix confirming the Office of Human Resources has received your request.

Congrats, you submitted an EAF! All intended parties will receive emails to approve the EAF automatically.

Approval Chain/Routing Slip

The routing slip for approvals is determined based on the action/reason selected on the EAF. Some EAFs will need review/approval from more stakeholders than others. All EAFs will first be reviewed by the HRIS Analyst team to ensure all data is included to make the changes in HRS.

 

The EAF is preloaded with specific approvals by various HR staff, Budget Office, or Affirmative Action depending on the action/reason listed. Below are the additional preloaded approvals in the routing slip and who is in each group:

  • EAF LOA Notify
  • BPL Budget Office
  • EAF HRIS Analyst
  • EAF Employment Specialist
  • EAF Benefits Specialist
  • HRP Team
  • EAF Affirmative Action

 

In addition to the preloaded routing slip, any stakeholders listed under the following fields may need to approve the EAF or receive an email notification about the changes:

  • Reports-to – approval needed
  • Division Head – approval needed
  • Business Manager name - approval needed
  • Add additional Business Managers (if applicable) – approval needed
  • Additional Stakeholders – notification only

 

 

Review Tasks/Previous EAFs

Under the EAF submitter tab, you will be able to see any EAFs that need action or submitted by the requestor.

 

Items listed under “EAF Task List” will show any EAFs that need action by that employee. In this field, you can approve, deny, push back, cancel, or edit the EAF in the list. Items listed under “My Submitted EAF Forms” show all EAFs that have ever been submitted by the employee. This includes EAFs that are still in the approval process or fully executed. Even if an EAF is still in the approval process you are not able to take action when it is selected from the “My Submitted EAF Forms” section. This is used as a reference section for EAFs only.

How to Edit/Cancel/Restart an EAF Request

Navigate to the BPLogix Homepage https://uwlax.bplogix.net/workspace.aspx

EAFs can only be edited/canceled/returned/approved if it is in your EAF Task List. Find the EAF you would like to take action on under the “EAF Task List” From here you have the option to approve, deny, return to submitter, or cancel an EAF request. A comment must be made if any option besides “approve” is selected.

  • Approve will move the EAF to the next group in the routing slip.
  • Deny will stop the entire process and the EAF can no longer be used.
  • Return to Submitter will return the EAF back to the initiator and can continue to be used. The initiator will receive the EAF and will have the option to resubmit the form.
  • Cancel EAF request will cancel the form and can no longer be used.

Emails are sent with any option to the initiator and the next employee to approve in the routing slip.

Frequently Asked Questions

  1. Does the initiator of the EAF receive notifications throughout the process?
    1. Yes! The initiator will receive emails when an EAF is submitted, denied, canceled, or completed.
  2. Where do the emails come from?
    1. All emails come from UWL Notify (bplogixnotify@uwlax.edu)
  3. What if I have more than one change for an employee, should I create multiple EAFs?
    1. No, you will only need one EAF per employee if all changes are for the same effective date. You can choose one action/reason that is being changed or you can choose “other-add comments”. All information that needs to be updated should be included in the EAF and a comment can be added to explain any/all changes being made.
  4. Does this count as an appointment document if I hire/rehire an employee?
    1. No, this form is just used to capture the approvals like the paper EAF. Once all approvals are complete, HR will create an appointment document for the employee to sign.
