Special Course Fee Form

The Special Course Fee Form is to be used when requesting the creation, modification, or elimination of a special course fee. Special Course Fees are required to follow UW System Financial Administration Policy G29 and the UW-L Special Course Fee Policy. All Special Course Fee requests must be submitted by February 15th for all terms for the upcoming academic year. Forms submitted after the due date will not be approved for that academic year.

Instructions to Submit the Form via BP Logix

1. Use the link above, or log in to BP Logix at
2. Click on Special Course Fee Form listed under "Forms I Can Submit". 
3. Fill out form, all required fields will be outlined in red
4. Once all required fields are completed, select Submit. 

5. Based on who you entered on the form in the Department Chair and Dean field, the form will now be routed to those individuals to approve/deny. 
If the Department Chair and Dean approve the request, the form then goes to the Budget Office to approve, followed by the Cashier's Office to complete your request. 
If at any step in the process your request is denied, you will receive an email as to why your request was denied.

The SFO Authorization Form may be completed using the PDF, linked above. A completed form may then be submitted to Business Services either by email attachment sent to or by in-person delivery to 125 Graff Main Hall.
