Special Course Fee Form
The Special Course Fee Form is to be used when requesting the creation, modification, or elimination of a special course fee. Special Course Fees are required to follow UW System Financial Administration Policy G29 and the UW-L Special Course Fee Policy. All Special Course Fee requests must be submitted by February 15th for all terms for the upcoming academic year. Forms submitted after the due date will not be approved for that academic year.

Upon opening the link to the Special Course Fee Form, the initiator of the form will be prompted to enter the roles of those individuals who will be involved in the completion of the form. Those persons who are not the initiator and are specified within the role fields will receive an email that contains a link to access the DocuSign form, and which will allow them to populate their assigned portion of the form. A description of the roles are as follows:
Initiator: This is a required role. This is you, the individual beginning the process of submitting this form to Business Services.
Academic Chair: This is a required role. This individual may also be the Initiator.
Academic Dean: This is a required role. This individual may also be the Initiator.
Budget Director: This role is prepopulated and is not modifiable by the Initiator.
Cashier's Office: This role is prepopulated and is not modifiable by the Initiator.
Once roles have been assigned by the initiator and they have selected Begin Signing in order to begin populating the form, those roles that were specified by the initiator will receive an email with a link to the form that will allow them to populate the portion associated with their role. Each person receiving such an email should populate those portions of the form that are assigned to their role, then select Finish.

The SFO Authorization Form may be completed using the PDF, linked above. A completed form may then be submitted to Business Services either by email attachment sent to firstname.lastname@example.org or by in-person delivery to 125 Graff Main Hall.
