Business Services Form - Credit Card Device Tamper Log

Credit card device tamper checks are to be completed once a day for every day that the device is used. The purpose is to establish a timeline for detecting fraudulent activity.

Policy

The use of this log is governed by the Business Services Policy for Accepting and Processing Payment Cards for University Business.

Instructions

Departments should print off a paper copy of the log to keep with their credit card reader. The individual in charge of using the credit card reader goes through the list of checks included on the form. Upon completion of the checks, they will sign their initials and the date that the check was performed. This review only needs to be completed on the days that the credit card reader device is used, and only needs to be done once each day. Send a copy of the fully completed log to Business Services at businessservices@uwlax.edu. A new log can be started after the old log has been submitted.
Any evidence of tampering should be reported to Business Services immediately by calling 608-785-8554.

Specific Field Instructions

The Department, Merchant ID, and Device Serial/Model Fields should be filled out immediately after printing the log to help ensure that the right device is consistently being reviewed.
-Department: This field is the department that owns and regularly operates the credit card reader.
 
-Merchant ID: The merchant ID is used to track payments that are processed through the credit card reader. A merchant ID is assigned to one or more credit card readers. Contact Business Services if you have questions on what the Merchant ID for your credit card reader is.
 
-Device Serial/Model Number. The serial and model numbers will be listed on the credit card readers. The serial number will typically be printed on the back of the device and will often be printed after the initials 'S/N:'. The model numbers typically appear on the front of the device. Most departments use Bluefin credit card readers. The Bluefin model numbers are going to either be A80 or A920.
