BUSINESS SERVICES FORM - PURCHASING CARD APPLICATION AND USER AGREEMENT - FORM # BUS-2024-001

The Purchasing Card Application and User Agreement Form streamlines the process of applying for a Purchasing Card by combining the three major steps into one easy-to-use form. The three steps include completion of (A) the mandatory PCard training course along with submittal of proof of completion, (B) the PCard Application, and (C) the PCard User Agreement.

Roles

The roles that will need to be entered in order to use the form are as follows:

Submitter: Initiates the request to become a PCard holder by:

  1. Completing the required Training Course (including the insertion of Course Completion documentation on page one)
  2. Filling in all required fields on page one of the Application portion of the form
  3. Reviewing the User Agreement on pages 2-3 and completing the electronic Cardholder Acknowledgment/Signature section on page three.         

Supervisor: Receives the completed Application and User Agreement and audits all supplied information for accuracy and completeness.  After providing a full review, the Supervisor will electronically complete the electronic Supervisor Acknowledgment/Signature section on page three. 

Upon completion of Supervisor's signature, the application packet will be forwarded to Business Services via DocuSign for processing.
