BP Logix - Change of Grade Form - Submitter

A step-by-step guide about how to submit a Change of Grade form in BP Logix.

1. Navigate to BP Logix at https://uwlax.bplogix.net/

2. Click on Change of Grade and/or Removal of Incomplete Form. 

3. Complete the Student Information section. Required fields are outlined in red.  

Email addresses: A student's email address can be found by hovering over the student's name in the class or grade roster. It shows as a pop-up message in the bottom left corner of the browser when you hover. Or you can click the Select box next to their name and click Notify Selected Student at the bottom of the roster. The next page will show the student's email, which can then be copied and pasted into the form.

If no email pops up on the roster, the student is not active. In that situation, please use the student's last known personal email address. If their personal email is not known, leave blank.

The email is for notification purposes only.

4. Complete the student's College/School, Level and Major/Program. 

College: The student's College/School of their primary major can be found on the class roster or grade roster. This will show whether or not the student is an active student.

  • The Class Roster is the 3 people icon next to the class in your Faculty Center.

  • The grade roster is the person at a board icon next to the class in your Faculty Center. 

Student Level: The student's level/class standing can be found on the class or grade roster if they are an active student. If that information is not listed, contact Kim Berg in Records for help filling out this information. Her email is kberg@uwlax.edu.

  • If they are freshmen, sophomores, juniors, seniors, or post-baac/second degree students, please select their Undergraduate Major, which is also listed on the roster. 
    • If that information is not listed, contact Kim Berg in Records for help filling out this information. Her email is kberg@uwlax.edu.
  • If they are graduate students in a program, please enter their program. 
    • If that information is not listed, contact Kim Berg in Records for help filling out this information. Her email is kberg@uwlax.edu.
  • Undergraduate and graduate special students are not in a degree seeking program, so no additional information is needed. 

5. Complete the Course Information and Change Information sections. 

6. Once all required fields are complete, select Submit at the bottom of the form. 

7. Once form has been submitted, it will route to the Department Chair and then the Academic Services Director, followed by Records and Registration for approval. If someone other than the instructor submits the form, it will go to the instructor first. 

Once all approvals are complete, a notification will be sent to the student, if an email address was provided, as well as the initial submitter of the form. 

If anyone in the process denies the request, the submitter will be notified via email.
